About The Role
 
Scope has over 250 Supported Independent Living (SIL) homes across Victoria caring for clients, often with complex care needs.  Full and part time opportunities are available for passionate, enthusiastic, and experienced senior registered nurses to join an exciting project focussed on supporting comprehensive and timely client assessment, care planning and education to service delivery teams in SIL’s.  
The role will work in partnership with the Project team, Customers, Disability Support Workers, families, and other Service Providers to optimise the safety, effectiveness and quality of client outcomes. 
 
The responsibility of the teams are to:
  • Engage positively with stakeholders and to ensure a robust Healthcare and Wellbeing Program for Scope Clients
  • Implement a standardised assessment and care planning process and procedures to optimise the safety and quality of care
  • Develop and deliver training modules for clinical activities including infection prevention and control and high intensity daily personal activities
  • Monitor, evaluate and improve services to ensure they meet the requirements of the NDIS Practice Standards and Quality Indicators and other relevant legislation.
The positions will be based at our main office at Hawthorn, however, staff have the ability to work remotely and be required to travel to SIL homes in metropolitan and regional areas to work with service delivery teams.  
 
About You
 
To be successful in this role, you will have: 
  • Demonstrated work experience in a senior nursing role in leadership, projects or education
  • Ability to communicate effectively with a range of stakeholders to achieve project outcomes
  • Capability to support and train Disability Support Workers to develop, implement, and evaluate health care management plans for customers with complex health needs
  • Commitment Co-design to ensure client goals, aspirations and choices are optimised
