About The Role

Scope has an exciting opportunity for Support Coordinators to come and join the expanding NSW team!

You will be mainly servicing customers in the New South Wales regions and building connections within these areas. Our flexible working from home approach means you’ll be servicing customers close to your home. 

  • Establish Support Coordination in your regions
  • Promote Scope brand awareness
  • Provide support co-ordination for NDIS participants who have complex and multiple disabilities
  • Facilitate implementation of all supports identified in a participant’s plan
  • Strengthen participants ability to navigate the NDIS and co-ordinate their supports
  • Empower participants to access and co-ordinate supports to enable each person to participate more fully in their community of choice
  • Develop strong local networks with service providers
  • Assist participants to identify the range of suitable supports and services in order to meet their individual goals identified in their NDIS plan
  • Work with the participant’s networks to ensure that they are effectively linked into funded supports and services
  • Work with the participant to link them into appropriate mainstream services such as housing, employment, education and health
  • Work with the participant to develop their skills and resilience

About You

  • Tertiary qualified, or equivalent in a discipline related to disability supports or 2 years experience working within the disability sector
  • Experience supporting adults and children with complex disabilities utilising a range of person centred service models
  • Experience in support planning and / or coordination
  • An understanding and sensitivity to the rights and dignity of people with disabilities inline with current legislation
  • An understanding of the NDIS and individualised funding
  • Knowledge of the disability service sector
  • Ability to work with minimal supervision
  • A high level of organisational and time management skills e.g. data collection, reporting

Please note, the successful applicant will be required to hold a NDIS Worker Screening Check Clearance and a Working with Children Check. If you have lived or worked for a continuous period of 12 months or more in another country within the last 10 years, a certified International Police Check from that country or countries will also be required.
